Role and responsibility
The unit watch officer or operations duty officer owns the OPREP-3 synchronous voice report at the moment of event awareness. The S-3 operations staff drafts the OPREP-3 message report per Enclosure 4 of MCO 3504.2A. The S-1 officer and adjutant support OPREP-3 SIR records retention and case file maintenance. The SARC supports OPREP-3 SIR sexual assault reports with the confidentiality rules per Paragraph 5a and Enclosure 7. The commander owns the OPREP-3 report content and signs supplemental reports.
Source documents flow from the event or incident notification, the unit muster sheet, the duty officer report, the safety board input (for mishaps), the SARC notification (for sexual assault), the law enforcement report (for criminal events), the medical record (for serious injury), and Enclosure 2 of MCO 3504.2A reportable events list. Outputs include the synchronous voice report log, the OPREP-3 message report, the supplemental reports, the retained record per SECNAV M-5210.1, and the unit OPREP-3 case file.
MCO 3504.2A is exempt from reports control per SECNAV M-5214.1 Part IV paragraph 7g. The order is the standing requirement.
Trigger events
- Significant event or incident reportable per Enclosure 2 of MCO 3504.2A.
- National-level interest event (OPREP-3 PINNACLE).
- Serious incident not at national-level interest (OPREP-3 SIR).
- Sexual assault per MCO 1752.5A SAPR Program (OPREP-3 SIR with Enclosure 7 confidentiality rules).
- Hazing event per Enclosure 6 sample format.
- Mishap per MCO 3040.4 (coordinate with the safety board).
- Things Falling Off Aircraft (TFOA) (notify Range and Training Area Management Branch).
- Operating under a sister Service component headquarters (submit per the receiving Service requirement, CMC PP&O as info addressee).
- OPREP-3 WHITE PINNACLE exercise (20-minute exercise window).
Processing workflow
- Identify the reportable event. Match against Enclosure 2 of MCO 3504.2A reportable events list per MCO 3504.2AMCO 3504.2A Paragraph 4. The list is a general guideline. When in doubt, submit a report.
- Determine the OPREP-3 category. PINNACLE (national-level interest, to NMCC), SIR (CMC through MCOC), sister Service variant (when operating under sister Service headquarters), or WHITE PINNACLE (exercise).
- Submit the synchronous voice report. PINNACLE within 15 minutes of awareness to NMCC. SIR within 30 minutes of awareness to MCOC. WHITE PINNACLE within 20 minutes for the exercise window.
- Draft the OPREP-3 message report. Use the Enclosure 4 message format. Reference Enclosure 5 for the sample SIR message, Enclosure 6 for the sample hazing message, and Enclosure 7 for the sample sexual assault message.
- Apply sexual assault confidentiality rules. Per Paragraph 5a, provide only rank or grade. Omit initials, last name, and last four of SSN. State "omitted to protect confidentiality" under the rank field when the rank or grade would compromise confidentiality.
- Apply EDIPI rules per DODI 1000.30. For non-sexual-assault reports, include only the EDIPI for personnel identification.
- Coordinate with operations, legal, and SARC. Operations leads the message drafting. Legal reviews the report content. SARC coordinates sexual assault case factual content.
- Route the message report. Submit to MCOC at the standard OPREP-3 message format. Include CMC WASHINGTON DC PPO as an information addressee when operating under sister Service component headquarters.
- Submit supplemental reports. Supplemental reports correct or add information to the initial report. Submit as required. Do not delay the initial report pending complete information.
- Retain the OPREP-3 report record. Submit retain all reports per SECNAV M-5210.1 and NAVMC DIR 5210.11E.
- File the OPREP-3 packet. Place the voice report log, the message report, the supplemental reports, and the supporting documentation in the unit OPREP-3 case file.
- Brief the commander. Present the OPREP-3 posture on a fixed cycle covering open SIR cases, recent closures, and emerging trends.
Systems of record and forms
Systems
- Marine Corps Operations Center (MCOC). Marine Corps OPREP-3 SIR receiving authority at HQMC.
- National Military Command Center (NMCC). OPREP-3 PINNACLE receiving authority for national-level interest.
- Navy Operations Center, Army Operations Center, Air Force Operations Center. Sister Service receiving authorities.
- NMCS command net chat room on DISA Defense Connect Online Jabber Chat (SIPR). Synchronous chat reporting channel.
- Unit OPREP-3 case file. Local archive of OPREP-3 reports retained per SECNAV M-5210.1.
Forms
- OPREP-3 SIR Message Format. Enclosure 4 of MCO 3504.2A.
- OPREP-3 SIR Sample Message. Enclosure 5.
- OPREP-3 SIR Sample Hazing Message. Enclosure 6.
- OPREP-3 SIR Sample Sexual Assault Message. Enclosure 7.
- OPREP-3 Reporting Contact Information. Enclosure 3 (MCOC, NMCC, sister Service contacts).
- Voice Report Log. Local log capturing the synchronous voice report content and time.
Common pitfalls
- Initial voice report delayed pending complete information. The 30-minute SIR timing window per MCO 3504.2AMCO 3504.2A Paragraph 4a(2)(a) takes precedence. Report what is known and supplement with additional information.
- Sexual assault report includes prohibited PII. The report exposes the victim or witness to confidentiality compromise. Apply the rank/grade-only rule per Paragraph 5a and Enclosure 7.
- Non-SAPR report omits the EDIPI. The OPREP-3 message fails to identify personnel per DODI 1000.30. Include the EDIPI on non-SAPR reports per Paragraph 5b.
- Sister Service operational control unflagged. The unit submits only to MCOC without notifying the sister Service component headquarters or without including CMC PPO as info addressee per Paragraph 5d.
- Records retention skipped. The OPREP-3 record is required to be retained per SECNAV M-5210.1 and NAVMC DIR 5210.11E. Archive every report in the unit OPREP-3 case file.
Decision points
- PINNACLE versus SIR. National-level interest drives PINNACLE routing to NMCC at 15-minute timing. Non-national-level interest drives SIR routing to MCOC at 30-minute timing. When in doubt on national-level interest, the originator escalates per MCO 3504.2AMCO 3504.2A.
- Marine Corps SIR versus sister Service variant. The unit reports per the operational control chain. Sister Service component headquarters operational control triggers sister Service variant per Paragraph 5d, with CMC PPO as info addressee.
- Synchronous voice versus chat. Voice is the primary synchronous reporting method. NMCS command net chat on DISA Defense Connect Online Jabber Chat via SIPR is the secondary method per Paragraph 4a(2)(a)1.
