MCO 1500.60A Para 4.b.(3)(n) requires the FPC SOP within 90 days of assumption. Para 4.b.(3)(b) requires monthly chairing. Para 4.b.(3)(l) requires Memorandum for Record when the CO does not chair. The SOP is your documented FPC discipline. The monthly chair is your personal time commitment. The MfR is the documented exception when you cannot chair.
What the FPC SOP Contains
MCO 1500.60A does not prescribe a single template. The Suicide Prevention guide and Mental Readiness Playbook V3 establish the working content set. A complete FPC SOP covers these eight sections.
1. Membership and attendance
- Voting members (chair, XO, SgtMaj, S-1, S-3, chaplain)
- Standing advisors (OSCAR Provider, FRO, SACO/UPL, SARC liaison)
- Topic-driven additions (Marine's small-unit leader, mentor)
- Attendance requirements and substitution rules
- Quorum minimum
2. Cadence and convening
- Standard monthly date and time
- Out-of-cycle convening triggers (suicide-related event, mass-casualty incident, command crisis)
- CMG coordination so the two forums do not collide
- Notification timeline for membership and agenda
3. Agenda structure
The Suicide Prevention guide implies a standing structure:
- Open with prior council action items
- New joins (90-day elevated rule applies)
- Marines flagged in CIRRAS at elevated and high risk
- Discharge or separation cases at risk
- Reintegration cases (post-attempt, post-treatment, post-court)
- Recurring CMG/SAPR overlap cases
- Cross-cutting trends or unit climate observations
- Action items and assignments
4. CIRRAS update discipline
- Update protocol after each council
- Owner per Marine's profile
- Completion audit at the next FPC
- Profile staleness rule (no profile older than 90 days)
5. Resource directory
- Local CCP, MFLC, OSCAR, chaplain contacts
- SAPR Hotline and SARC
- Military Crisis Line (988, press 1)

Why "Available to All Assigned Marines" Matters
Per MCO 1500.60A Para 4.b.(3)(n), the SOP is available to all assigned Marines. Not only leaders. Not only the chain.
The intent is transparency: Marines know how the force preservation forum works, who attends, what is documented, and how a flag in CIRRAS gets handled. Transparency reduces stigma.
This does not mean Marines see other Marines' CIRRAS profiles. Profile access remains restricted. But the SOP itself - the process - is published.
Practical formats:
- Posted on unit SharePoint or shared drive
- Briefed in newcomer orientation
- Referenced in command philosophy or commander's intent letter
- Included in S-1 onboarding package
Meeting Cadence Discipline
The monthly minimum is the floor. Many units run the FPC at higher frequency during sustained operational tempo.

Cadence flexibility is fine. Monthly minimum is non-negotiable.
Attendance Discipline
Two anchors govern attendance.
- CO chairs personally. Non-delegable per Para 4.b.(3)(b).
- MfR for absence. Required per Para 4.b.(3)(l), retained 3 years.
What the MfR looks like
A complete MfR captures:
- Date and time of the FPC
- Reason CO was unable to chair (operational commitment, leave, medical, exigent absence)
- Who chaired in lieu (typically XO)
- Whether the in-lieu chair had quorum
- Significant decisions made
- CO acknowledgement and signature
What does NOT qualify as MfR substantiation
- Routine schedule conflict ("had a meeting")
- Predictable command obligation schedulable around the FPC
- Habitual pattern of XO chairing
If the MfR file shows a pattern, the practice is delegation by another name. MCO 1500.60A treats the pattern as a finding.
Documentation Set Per Council
After each FPC, the documentation record includes:
- CIRRAS profile updates for every Marine reviewed
- Council minutes with attendees, Marines covered, decisions, action items
- MfR if CO was unavailable
- Action item tracker carried forward to next council
- Resource referral log if new referrals were made
The Suicide Prevention guide's recurring update direction makes the CIRRAS profile update the single most important post-council action. Profile completion is the test.
Council Minutes - What They Look Like
Minutes are not transcripts. The standard captures:
- Date, time, location
- Attendees by role
- Marines reviewed (by name or by reference number per local protocol)
- For each Marine: risk indication change, action items, owner, due date
- Cross-cutting observations (unit trends, resource gaps, climate touchpoints)
- Action items from prior council and completion status
Minutes are sensitive. Distribution follows the same need-to-know discipline as CIRRAS profile content.
How FPC Coordinates with Other Forums
The FPC does not stand alone. Three overlapping forums need coordination.

Run them on a coordinated calendar. A council schedule placing FPC and CMG on the same day, back to back, is efficient when membership overlaps.
Common Failures
- No SOP at 90 days. Direct MCO 1500.60A Para 4.b.(3)(n) violation.
- SOP exists but is not available to assigned Marines. Compliance technicality with substantive failure.
- XO chairs habitually. MfR file shows the pattern. Finding under MCO 1500.60A.
- Council minutes filed but no CIRRAS update. Documentation theater. The CIRRAS update is the operative record.
- MfR is one line: "CO was busy." Insufficient. The MfR captures the substantive reason.
- Profile-level CIRRAS access by civilian staff. Direct violation of Para 4.b.(3)(a).
- FPC running monthly but with rotating attendance. Continuity breaks down. The same set of leaders should see the trajectory of the same Marines.
- No quorum rule. Council convenes with three people present. Cannot validate risk determinations.
Annual SOP Review
The SOP is reviewed annually at minimum. Trigger events for off-cycle review include:
- New CO assumes command (the new CO publishes their own SOP within 90 days)
- New MARADMIN or MCO updates the FPC requirements
- Significant incident reveals a gap in the procedure
- Audit finding from higher headquarters or IGMC
Mark the next review date on the SOP itself. A static SOP from three years ago is presumptively stale.
