Page 11 Entries - Commander Authority
Page 11 of the Service Record Book (SRB) is the administrative record for adverse and counseling entries. The CO directs entry, signs significant items, and reviews routing through S-1 and IPAC. This page covers what goes on Page 11, what does not, and the rights chain for the Marine.

What Page 11 Is
Page 11 is the SRB section for administrative remarks. The page captures counseling, adverse actions short of NJP, NJP results, awards, achievements, formal warnings, and other items requiring permanent documentation in the Marine's record.
Page 11 is the most-litigated SRB page. Marines challenge Page 11 entries at promotion boards, at separation boards, and in retention decisions. Sloppy Page 11 entries weaken disciplinary records and create rebuttal grounds.
When You Direct an Entry
The CO directs Page 11 entries on items requiring permanent record. These include:

The CO is not required to personally write every Page 11 entry. S-1 drafts. The CO directs the entry, reviews the draft, and signs where authority requires.
When You Sign Personally
You sign personally for:
- NJP results (you imposed the NJP)
- Administrative reduction outside NJP
- Adverse action entries supporting separation processing
- Significant adverse entries you specifically directed
S-1 signs less significant counseling entries with delegated authority. The CO sets the threshold in unit policy.
The Marine's Rights
Page 11 entries trigger procedural rights for the Marine.
- Acknowledgement. The Marine reads the entry and signs to acknowledge receipt. Acknowledgement is not agreement. It is documented awareness.
- Rebuttal opportunity. The Marine has the right to submit a written rebuttal within the timeframe set by MCO P1070.12. The rebuttal becomes part of the record.
- Refusal to sign. If the Marine refuses to sign, the entry is annotated as such. The substantive entry stands. A witness verifies the refusal.
- Appeal through the chain. Marines retain the right to appeal substantive entries through the chain of command and via Article 138 UCMJ complaint where applicable.
Skipping the acknowledgement step is the most common procedural defect. Entries on file without Marine signature or refusal-to-sign annotation are challengeable.
What Does NOT Go on Page 11
The IRAM is explicit on exclusions.
- Hearsay or unverified allegations
- Off-duty conduct without command nexus, unless reportable per substance abuse, OSTC covered offense, or other specific authority
- Counseling sessions with no documented deficiency or expected change
- Counseling delivered before the Marine had opportunity to know the standard
- Adverse opinion absent supporting fact
Page 11 documents conduct and counseling. It does not document the chain's opinion.
Routing Through S-1 and IPAC
After signature and Marine acknowledgement, S-1 routes the entry:
- Original to OMPF via IPAC processing
- Copy to SRB
- Copy to Marine
- Coordination with promotion records if the entry affects promotion eligibility
- Coordination with reenlistment records if the entry affects retention eligibility
A Page 11 entry not routed to OMPF exists only on the unit's copy. At separation processing, the OMPF is what travels. Routing discipline is on the S-1 but the CO confirms.
Page 11 in Disciplinary Context
When misconduct is alleged but not yet adjudicated, the Page 11 entry pattern depends on whether the case proceeds.

The deferred pattern on OSTC covered offenses is critical. A Page 11 entry prejudging OSTC disposition risks tainting the proceeding. Coordinate with SJA before signing.
Common Failures
- Page 11 entry without Marine signature or refusal annotation. Procedural defect. Marine challenges later.
- Hearsay or unverified allegations on the entry. Rebuttal grounds the Marine wins on review.
- No documentation of the standard the Marine failed to meet. Counseling without a measured deficiency. Entry struck on appeal.
- NJP-related Page 11 not signed by the CO who imposed NJP. Authority defect.
- Pattern entries written without prior counseling. A Marine has no documented warning. Pattern claim fails.
- OSTC covered offense entry made before OSTC disposition. Prejudgment. Tainted proceeding.
- Entry not routed to OMPF. Page 11 exists at unit level only. Lost at separation processing.
Decision Framework Before You Direct an Entry
Before directing the entry, ask:
- Is this conduct or counseling permanent-record-worthy?
- Has the Marine been counseled previously on this standard?
- Is the standard documented and applicable to the Marine?
- Is the alleged fact verified?
- Will the entry support a future personnel decision (FITREP, separation, denial of reenlistment)?
- Is there an OSTC overlap requiring SJA coordination first?
- Does the entry meet IRAM signature and content requirements?
A "no" or "unclear" on any of these is a sign to refine the draft before signing.
Coordination With Related Roles
- S-1 admin drafts the entry, prepares the routing package, and processes through IPAC.
- SJA advises on entries with potential legal implications.
- SgtMaj / SEA counsels on the pattern and ensures the small-unit leader has done the underlying counseling.
- Marine acknowledges the entry, retains rebuttal rights, and receives the routed copy.
