Scope
NAVMC 321M, Agreement to Extend Enlistment, revision 10-16, electronic fillable (EF). Three pages: a Privacy Act statement, the agreement itself with items 1 through 19, and a completion instructions page. The governing order printed on the form is MCO 1040.31. Audience
Admin specialists prepare the form and run the MCTFS reporting. Enlisted Marines extending for temporary limited duty or an IDES referral sign it before the Commanding Officer. The first extension processes at unit level through IPAC, second and subsequent extensions route to MMSR-4.
Purpose printed on the form
Records voluntary extensions of enlistment into the U.S. Armed Forces. The completed form becomes a permanent part of the Marine's Enlisted Master File and Field Personnel File and files into the OMPF. Privacy Act authorities are 10 U.S.C. 5013 and 5041, 42 U.S.C. 10606 as implemented by DoDI 1030.1, E.O. 9397, and SORN M01070-6.
When Marines use it
The revision 10-16 form centers on medical extensions. Item 17 requires one of three attachments: a NAVMED 6100/5 for a temporary limited duty (TLD) extension, the Joint DoD IDES referral form for a referral into the Integrated Disability Evaluation System, or a Line of Duty benefits authorization letter. Extension terms run up to 6 months for TLD or 9 months for an IDES referral.
Completion rules from the instructions page
- Dates enter in YYYYMMDD format, for example 20101020. The sworn date in item 15 enters in all capital letters with the month spelled out, for example 22 MARCH 2016.
- Terms of enlistment and extension enter in both word and figure.
- Time lost to be made good enters by word, with ZERO when none exists.
- Item 12 states the specific purpose and authority, for example assignment to temporary limited duty under Commanding Officer authority or referral into the IDES under MEB convening authority. Include the CMC authority DTG or PAR and reference. Never enter the word None.
- Lump sum leave. Marines sell back leave in conjunction with the first extension. A Marine declining the sale on an extension taking effect within 30 days of signature enters I DO NOT DESIRE PAYMENT OF MY LUMP-SUM LEAVE on the form.
- The Marine signs full first, middle, and last name and executes the agreement before the Commanding Officer.
Routing and approval
Once effective, only CMC MMSR-4 or CMC WWR RMED cancels the extension, upon receipt of the PEB index to adjust PRR and EAS. Item 18 approval by CMC MMSR-4 or CMC WWR RMED applies only to second and greater medical extensions beyond the existing contractual EAS per MCO 1900.16 paragraph 8105.3. The reporting unit runs TTC 116 00 with EXTENL, months, and EXT entries, then files the form into the OMPF.
